Consent to the Processing of Personal Data Obtained in the Form

of a Copy of an Identity Card or a Passport for Purposes of Registration for the Preparatory Courses for Studies in the Czech Language and for the Intensive Czech Language Courses

I consent to the taking of a copy of my identity card or a passport and the processing of my personal data contained in it.

The consent is given for the purpose of the proof of validity of my identity card or passport at a time of filing an application form for the Preparatory Courses for Studies in the Czech Language and for the Intensive Czech Language Courses.

The consent is valid provided my personal data is processed only to the extent necessary for the purpose of the processing described above and in accordance with applicable legislation.

I consent to the keeping of a copy of my identity card or passport by the University of West Bohemia in Pilsen for a period of time:

I consent to the disclosure of the copy of my identity card or passport by the University of West Bohemia in Pilsen:

The University of West Bohemia in Pilsen is authorized to use the copy of my identity card or passport only in accordance with the purpose specified above or for the legitimate need of state control bodies and bodies responsible for criminal proceedings.

The University of West Bohemia in Pilsen is further authorized to submit the copy of my identity card or passport to third parties that cooperate with the University of West Bohemia in Pilsen to achieve the primary purpose for which this consent is granted. With such entities, the University of West Bohemia in Pilsen undertakes to conclude a contract providing for the same conditions for the processing of my personal data. 

Data will be processed in accordance with the relevant legislation on the protection of personal data and with Regulation 2016/679 of the European Parliament and of the Council (EU) of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and on repealing Directive 95/46/EC (general regulation on the protection of personal data).

I have been advised that the provision of the data is voluntary. I have furthermore been advised, in compliance with the relevant legislation, that:

• I have the right to withdraw my consent without giving any reason,
• I have the right to access these data and the right to amend them,
• I have the right to ask for the deletion of these data if they are processed in violation of the protection defined by the applicable legislation, or the processing is inconsistent with this consent, or the consent has been withdrawn.

I have also been advised that I can exercise these rights by sending a written request to the address of the University of West Bohemia, Univerzitní 8, 301 00 Pilsen.

I acknowledge that the withdrawal of this consent may affect the achievement of the purpose for which this consent was given, unless that purpose can be achieved otherwise.
